A Behavioral Health Home service is available to eligible patients that already be receiving services from a Psychiatric Rehabilitation Program worker, a therapist and a Psychiatric Prescriber.

A Health Home Nurse can coordinate appointments, help with smoking cessation, diabetes control and transition care.
