New Patient Registration
To expedite your enrollment, please be sure to have the following while registering:

Photo ID

Active Insurance Card. If you do not have insurance, please indicate Self-Pay

Completion of the consent for treatment, release, and History Questionnaire with your reason for requesting treatment.
IF APPLICABLE, PLEASE ENSURE YOU ALSO HAVE

Proof of guardianship or other medical decision-making ability for the patient.

Contact information for any other doctors, agencies (e.g. PRP, DJS, wraparound, etc.), or providers involved in your care to complete the Release of Information form.

Payment method for co-payments (if needed).
Once we have all the required information, we will contact you for an appointment.

Community Behavioral Health is a Joint Commission accredited mental health clinic that meets the standards and requirements of current best practices.
PROCESS OF ENROLLMENT

Complete an initial biopsychosocial evaluation with the therapist

Complete an initial psychiatric evaluation with a physician. The doctor may request:

Lab work (if not done in the last 3-12 months)

Additional neuropsychological testing

Outside records from previous mental health providers or others

Complete a treatment plan to outline your goals and objectives in care with a therapist

Schedule recurring appointments with a therapist and/or doctor if needed.
BEST PRACTICES
Our therapists and doctors collaborate in all aspects of treatment planning which prevents us from offering medication-only services in line with evidence based practice for best outcomes.
Community Behavioral Health's psychiatrists may continue previous treatments from outside providers or will make evidence-based treatment recommendations meeting current clinical practice guidelines.
Our expert team will discuss with you if you're eligible for our Psychiatric Rehabilitation Program (PRP), Intensive Outpatient Program (IOP), Behavioral Health Home (BHH) or Targeted Case Management; we do not guarantee that you will see all of the mentioned providers in the first and consecutive appointments.
You may be asked for a urine and blood sample periodically and on the first day you attend the clinic, we reserve the right to offer a re-schedule if you're unable to be tested.
Please be aware if you are on benzodiazepines or other controlled substances that are clinically contraindicated for your condition, the team will work with you to ensure a safe taper to reduce your risk of dependence. Your treatment team is not obligated to continue a medicine they determine to be a risk to your health.
DISCHARGE POLICY
Consecutive missed appointments with either a therapist and/or doctor without prior notice will eventually lead to patient discharge.
Violation of clinic policy such as inappropriate comments or behavior made by a patient or a patient's relative to any staff, threats, or destruction of property can lead to immediate discharge.
