CBH provides this service in the mid-shore counties only: 

  1. Caroline County 

  2. Dorchester County 

  3. Kent County 

  4. Talbot County 

  5. Queen Anne’s County

Eligibility

CBH offers case management services to individuals who have been identified as: 

  1. Having a serious and persistent mental health needs 

  2. Are at risk of homelessness or hospitalization.

  3. Medicaid / Medicare Eligible
